J&J Q3 2026: Lower Quarterly Earnings, Higher Full-Year Guidance
Official release: 2026-10-13 · Research cutoff: 14 October 2026, 23:20 UTC · Third quarter ended September 27, 2026 versus September 28, 2025; full-year 2026 guidance separately
Johnson & Johnson raised its full-year adjusted earnings guidance while reporting lower quarterly earnings on both GAAP and adjusted measures. Those statements answer different questions. The quarter describes results already recorded; the revised guidance describes management’s current annual expectation. A GAAP-to-adjusted reconciliation explains another distinction: which costs and gains are included in each earnings measure.

| Q3 2026 earnings reconciliation | USD millions |
|---|---|
| GAAP net earnings | 210 |
| Intangible asset amortization | +1,001 |
| Litigation related | +4,289 |
| In-process R&D impairments | +254 |
| Restructuring related | +158 |
| Acquisition, integration and divestiture related | −52 |
| Securities losses/gains | +10 |
| Orthopaedics separation related | +301 |
| Tax legislation and other tax related | +168 |
| Adjusted net earnings | 6,339 |
The aggregate gap is $6.129bn. It should not all be described as a talc charge: the litigation adjustment is one category, with other additions and a negative adjustment also contributing. An accounting reconciliation category is not a cash settlement amount or a finding about legal liability.
Check every step, including the negative adjustment
210 + 1,001 + 4,289 + 254 + 158 − 52 + 10 + 301 + 168 = 6,339. The six smaller adjustments total $839m. Omitting the negative $52m because the other adjustments are positive would overstate the result. Use the signed amounts and the after-tax basis consistently.
Adjusted earnings also declined
Reported sales increased 5.9%, but adjusted net earnings fell from $6.801bn to $6.339bn. Adjusted diluted EPS declined 7.5%. Consequently, removing special items does not turn this into a quarter of year-on-year earnings growth.
J&J states that adjusted earnings exclude intangible amortization and special items. It also says adjusted EPS still reflects the Firefly acquisition and Sail collaboration impact. “Adjusted” therefore should not be read as “every deal-related effect removed.” The definition and the actual reconciliation matter more than the label.
Interpretation: GAAP and adjusted results are complementary views. The former records the included accounting consequences; the latter applies management’s exclusions. Neither should be treated as the uniquely correct number for every purpose. Excluding an expense from an adjusted measure does not prove it has no economic cost.
An annual forecast can improve after a weaker quarter
The full-year adjusted EPS guidance midpoint increased by $0.06, from $11.04 to $11.10. The estimated reported-sales midpoint rose from $101.1bn to $101.2bn. These are revisions to earlier forecasts, not percentage changes in the latest quarter’s actual results.
Conceptually, a forecast revision compares today’s expectation with a previous expectation. A year-on-year quarterly comparison instead uses last year’s realized quarter as its baseline. Because the baselines differ, one comparison can improve while the other deteriorates. The headline becomes confusing only if those comparisons are silently combined.
Does the raised guidance establish future GAAP profit?
No. The EPS guidance here is adjusted and forward-looking. J&J does not provide the corresponding forward-looking GAAP reconciliation because outcomes such as litigation and unusual items cannot be predicted with reasonable certainty without unreasonable effort. The guidance is management’s estimate, not a guaranteed result or a substitute for reported earnings.
A three-question reading checklist
For each number, ask: Is it an actual result or a forecast? Does it cover a quarter or a full year? Is it GAAP or a defined adjusted measure? Keep those answers attached to the number when comparing headlines, building a chart or discussing a change in expectations.
Also preserve the fiscal dates. This quarter ended 27 September 2026; the comparable quarter ended 28 September 2025. The 13 October release date is when the information was announced, not the period in which the sales and earnings arose.
